It happens mostly when you do not select client.exe on "Client Options" section. It must be client.exe not uo.exe or another executable file.
On "Ultima Online" section just select your UO folder.

Hi, are you feeding it manually or via macro?
In case it is via macro, you should use feed command, as documented here:

I was doing it manually, but I will take a look at that.
